HCBS Director

  • HAB Daily
    • The HAB Daily program, otherwise known as “Group Homes” or “Adult Habilitation” are provided in supportive, home-like environments. We provide services for individuals eligible to receive HCBS Habilitation, as well as Supported Community Living through the Intellectual Disability and Brain Injury HCBS Waivers. Each member participates in daily activities designed to meet their developmental, behavioral, and social needs, focused on the development of independent skills.
    • Eligibility: Individuals eligible for this program must be 18 years of age or older and on the HCBS waiver through Iowa Medicaid. Due to HIPAA, individuals on the Sex Offender Registry are ineligible for services in the HAB Daily program.
  • HAB Hourly & Supported Community Living (SCL)
    • HAB Hourly is similar to our HAB Daily with the exception being location. We meet our members in their homes in the community. From there, we work on the goals created in the individualized care plan meetings by the members. This program is based on relationship building, community, and independence.
    • Eligibility: Individuals eligible for this program must be 18 years of age or older, on the HCBS waiver through Iowa Medicaid, and have an IHH Care Coordinator or MCO Case Manager.
    • The SCL program works with individuals on the Intellectual Disability (ID) and Brain Injury (BI) waiver through Iowa Medicaid. SCL members create goals in the individualized care plan meeting with their care team. Services are had in their home or in the community depending on the needs of the member.
    • Eligibility: Individuals eligible for this program must be 18 years of age or older, on the ID or BI waivers through Iowa Medicaid, and have an IHH Care Coordinator or MCO Case Manager.
