The Woodward Community Based Services behavioral health continuum allows us to come alongside a struggling child, youth or adult and ensure they receive the treatment, care, and therapy required in order to strive toward their best potential and set them up for a lifetime of success.
Woodward Community Based Services has built and maintained relationships with community partners and agencies, such as Departments of Human Services, Departments of Juvenile Justice, Schools, Managed Care Organizations, and many others. We are continuously looking to enhance our partnerships and foster new relationships with the mission to help those in need of behavioral treatment to reach their best potential and provide them with the tools to lead productive lives.
Referrals can be made directly to your local Woodward Community Based Services program representative or contact us to learn more.

In order to expedite your referral, we may request the following information:
- Demographics
- Background information
- Psychiatric/psychological evaluations (if applicable)
- Court documents (if applicable)
- Other supporting documentation
