Residential Services
Woodward Community Based Services’s Adult Habilitation Services are provided in supportive, home-like environments. We provide services for individuals eligible to receive HCBS Habilitation, as well as Supported Community Living through the Intellectual Disability and Brain Injury HCBS Waivers. Each home resident participates in daily activities designed to meet their developmental, behavioral, and social needs, focused on the development of independent skills.
Services Provided
- Person center interventions developed by the individual and their interdisciplinary team
- Transportation
- Staff supervision
- Community integration activities
- Participation in individual care plan meetings, and training events directed toward individual service objectives.

- Adults, ages 18 and older
- Individuals eligible for HCBS Habilitation Services through Iowa Medicaid Enterprises
- Individuals eligible for Social Security Disability Income

Home-Based HAB Hourly
- Our HBH Hourly program is very similar to our daily program with the difference being location. We meet clients in their homes within the community. From there, we work on the client’s goals that are created in their individualized treatment plans centered around the strengths and needs of the individual. Self-care and safety plans are created with the clients alongside their team. A large focus of the HBH Hourly program is community integration through job training programs, volunteer opportunities, as well as accessing community resources including food/hygiene pantries and assistance with signing up for financial assistance program.

SCL Services Program
- The WCBS Supported Community Living (SCL) program works with individuals on the Intellectual Disability (ID) and Brain Injury (BI) waiver. SCL clients create individualized treatment plans with their teams centered around their strengths and needs. They then meet with WCBS staff in their home or in the community to work on the goals. The WCBS SCL program assists with skill-building, access to community resources, as well as community integration.
