Juvenile Tracking, Monitoring, and Outreach Program

Woodward Community Based Services Tracking, Monitoring and Outreach Program provides services to youth who are involved in Juvenile Court Services. We have programs in the 2nd, 4th and 5th Judicial Districts. The primary goal of this program is to assist youth in successfully remaining within the community by assuring daily accountability of the youth, in turn reducing further law violations. Services promote responsible behavior and accountability in the assigned youth, prevent further delinquent behaviors and reduce the risk to the community. This program provides supervision and structure, a link between the youth and their Juvenile Court Officer, referrals for additional services when needed, teaching and modeling of appropriate skills, crisis intervention and application of rewards and sanctions in collaboration with the Juvenile Court Officer.

Scope of Services: Tracking, Monitoring and Outreach Services include:

  • Intensive supervision of youth under the guidance of Juvenile Court Services
  • Developing a treatment plan in conjunction with the referring JCO within 30 days of referral
  • Assisting clients in the completion of probation requirements
  • Addressing delinquency risk factors as directed by the JCO, including violent behavior, anti-social attitudes, lack of compliance with rules, oppositional behavior, gang activity, mental health problems, substance abuse, and more
  • Crisis intervention
  • Referral to appropriate community services
  • Teaching/modeling skills for both parents and clients so that they can become self-sufficient
  • Creative use of sanctions/rewards to address resistance and progress
  • Detention Alternative

Client Profile

  • Males and Females
  • Ages 16-24
  • Juvenile Court involved (Polk County) within 5th and 2nd JCS Districts
  • Currently in or has been in an out-of-home placement

Community-Based Intervention (CBI)

Woodward Community Based Services Community Based Intervention Program provides services to youth who are involved in Juvenile Court Services located in the 5th District, specifically targeted towards Informal Probation. The primary goal of this program is to reduce youth’s risk of future offending by mediating their identified risk factors through targeted interventions and balanced supervision. This program is developed with a therapeutic philosophy that strives to bring youth’s accountability and behavior change by facilitating personal youth development. Methods include; targeted interventions in the form of skill building, activities to develop prosocial thinking and relationships, and refer additional services such as; substance abuse treatment, community service programming, and mental health services when needed. Case managers monitor the completion of the youth’s requirements referred by Juvenile Court Officers, conduct home and community based visits, drug testing when designated, teaches and models appropriate skills, crisis intervention, and report monthly regarding progress related to their individual goals and recognized requirements.

Scope of Services: Community Based Intervention

  • Supervision of youth on Informal Probation under the guidance of their designated Juvenile Court Officer/Central Intake Officer
  • Developing a treatment plan in conjunction with the referring JCO/CIO within 30 days of referral
  • Complete individual case notes and monthly progress reports per youth
  • Assisting youth in the completion of probation requirements
  • Addressing delinquency risk factors as directed by the JCO/CIO including; anti-social behavior, attitudes and beliefs, relationships, mental health, substance abuse, skills, aggression, and current living arrangements
  • Crisis intervention
  • Random drug and alcohol testing when designated
  • Referral to appropriate community outreach services
  • Plans and prepares group sessions once a month for referred youth
  • Teaching/modeling skills for both parents and youth so that they can become self-sufficient
